Overview of the Role:

As a Registered Dietitian, you will play a vital role in enhancing the health and well-being of PACE participants by providing expert nutrition care and education. You will conduct nutritional assessments, develop individualized care plans, and collaborate with an Interdisciplinary Team to ensure optimal health outcomes. Your expertise will directly contribute to the company's mission of delivering superior health care services.

Responsibilities:

• Conduct comprehensive nutritional assessments, collecting and documenting patient data.
• Develop and implement tailored nutrition care plans aligned with industry standards.
• Implement effective nutrition management in coordination with primary care physicians.
• Deliver in-service education to participants, families, caregivers, and staff.
• Evaluate participant progress by monitoring nutritional intake and making necessary adjustments.
• Participate actively in interdisciplinary team meetings to enhance patient care planning.
• Take initiative in leadership roles within department functions and quality improvement processes.
• Engage in additional projects and duties as assigned, contributing to overarching team goals.

Requirements:

• Bachelor’s degree in Nutrition from an ADA-accredited institution.
• Minimum 1 year of experience as a Dietitian; clinical dietetics for geriatric populations is preferred.
• Registered member of the American Dietetic Association (ADA) and registered by the Commission on Dietetic Registration.
• CDE status preferred or eligibility within 24 months.
• Experience with frail or elderly populations preferred; training provided if needed.
• Reliable transportation, valid driver’s license, and required auto insurance.
• Medically cleared for communicable diseases with up-to-date immunizations.
• Successful completion of a comprehensive background check.
